Description

Use six short questions to record an external quality assessment (EQA) result and prepare a review. Describe the result, the comparison target, how it was assessed and what you need to ask next.

What is included

  • An editable Word sheet designed to fit on one page.
  • A matching one-page printable PDF.
  • A completed one-page fictional example showing that a result can match other labs using the same method and still differ from another comparison target.

How to use it

The sheet uses everyday questions and gives space for short notes. You can copy information from the report and write “not known” where you need help. An experienced colleague can then help review the gaps.

Designed for staff who are new to laboratory quality as well as experienced staff who want a compact record. You do not need to fill every gap yourself.

The sheet helps prepare a review. It does not decide whether patient results are acceptable or replace your laboratory’s response procedure. The example’s values and limits are invented and must not be used as operating limits.

Read the EQA guide for more explanation. You can separately choose the free introductory email course. The Laboratory Quality A–Z explains unfamiliar terms.
