Description

Use six short questions to record an internal quality control (IQC) problem. IQC uses control samples to watch for changes in a test. Follow your laboratory’s response procedure first, then use the sheet for notes and discussion.

What is included

  • An editable Word sheet designed to fit on one page.
  • A matching one-page printable PDF.
  • A completed one-page fictional example about a control result flagged soon after a new vial was opened. The cause is still unknown.

How to use it

Record what happened, what else has been checked, recent changes, available records and questions for an experienced colleague. Short notes are enough. Write “not known” where you need help.

Designed for staff who are new to laboratory quality as well as experienced staff who want a compact record. You do not need to fill every gap yourself.

The sheet helps prepare a review. It does not choose QC rules, diagnose a fault, give corrective instructions or decide whether patient results can be released. Completing it must not delay the laboratory’s response to a problem.

Read the IQC guide for more explanation. You can separately choose the free introductory email course. The Laboratory Quality A–Z explains unfamiliar terms.
